Phase 2: Diagnostic |

The Diagnostic Phase allows the team to define the current state through quantitative and qualitative analyses as well as identify issues, understand root causes, and prioritize improvement opportunities. The Process Improvement Program (PIP) aims to complete this phase in eight weeks. Depending on your hospital’s state of readiness and competing priorities, however, you may find that more or less time is appropriate. During the Diagnostic Phase, you will understand the patient experience from arrival in the ED to discharge from the inpatient unit. You will assess the potential improvement impact of each opportunity identified as well as assess the team’s current skill set relative to the capabilities required to achieve the desired results. It is critical that this phase remain objective and based on objective data, rather than anecdotes that support predetermined views of what needs to be done. This phase is the most technical and utilizes some complex tools. Remember, as you go through the tools, not every tool outlined in this section will be appropriate for your hospital.
The Diagnostic Toolkit download (top right corner) includes workplans and all the necessary tools, guidelines and checklists to complete this phase — feel free to explore these components individually by clicking on the corresponding links below.
